If you are lucky enough to of got a green card at least get over there and get use it. If you don't spend at least 3mths in the US within your first year of winning it you will lose it. See how you like it if its the real thing with your girl you will make it work and between you, you can see each other a few times a year not ideal but it may only be for a year or two. At least if you decide to go back to OZ to be with her you have a green card and can both head over there later... keep your options open y'all!

Seriously though, I've worked in ad agencies in Sydney and Chicago/NYC, Sydney is just as good. There are agencies like hostville.com.au which when I was there had americans queing up to get there.
You'll have more chance of rising the ladder quicker in Sydney, then in 5 years time you could choose to move then.
She can do medicine in sydney if she had to, and you can have you career. 4 years from now both go to NY.

I would move to NYC personally. If I didn't, I would resent her for not doing so and always will wonder what would have happened. You are a lucky winner of greencard. An opportunity like that doesn't come that easy, right?
30 is still young to make a huge change in your life and do something that'll surprise yourself and be proud of.
