Think about it logically
She does med school in Canberra, you at a large global agency in Sydney (they are all there, TBWA, Leo Burnett, DDB etc) whilst she's at uni you have 4 years to get up the ladder. In four years you say I want to work in the US and if you're any good they'll transfer you for fear of loosing you from the company.
Meantime you see each other every weekend, Fuck where would you want to be at weekends? On the beach or in Canberra. You'll earn enough to help out with train fares, she has very long holidays anyway so you're only long distance half the year. End of Med school you both aim to work in the US as man and wife they'll pay for both of you to relocate.

long-distance relationships really are almost impossible. they rarely work out and when they do it's usually because the time apart isn't too long, so there's something to look forward to. 4 years is an eternity in that respect.
in 4 years she'll be out of her early 20s and into her late. a LOT of things change for women during that time period, so what she wants now will probably change.

Telecommute my man. Set up all your appointments in Sydney for like 1-2 days a week. Make the trek to Sydney, and then go back to Canberra when you've met with everyone. May not be the most desirable of set ups, but I used to commute 4hrs. a day for the sake of being happier when I was home.

I would go to NY. It's a tough choice that only you can make, but I don't think you'll regret it. You WILL regret it if you let someone hold you back, regardless of whether you love them or not.

I haven't read any of the other replies, but I think you already made up your mind when you didn't like the idea of moving in the first place.
I am probably gonna step on a lot of toes here and god knows I'm probably the biggest spokesperson on this site for career over ALL, but being really in love with someone, it's a no-brainer for me. I'd move, for her. Catastrophic economical and social decision, not to mention insanely idiotic in every way - but I would.
On the contrary, if she was feeling the same way - she wouldn't move 4 hours away in the first place. So in your case, I suppose not.
