What do you use to keep track of hours worked on a project for small projects that you bill by the hour?
Do you just use your clock or do you have an app or something?

I just count the seconds out loud while I'm working.
ONE. TWO. THREE. FOUR. FIVE. SIX... etc. Until I'm done with the project.

There are many options. It depends on specifics... on the exact needs for you...
I use Harvest with the people I work with. It's a paid service, but it does the job and allows us all to track our hours on a single item/project. To us harvest it's really helpful since we are a group of people. Might be a bit too much if you it's just you and don't need to have multiple people tracking their progress. I find Harvest beneficial when working with a group of people on projects of considerable size, like 100+ hours.
For my freelance stuff I use Billings. It's very simple. You install it on your machine, track your time right here as you work, you can organize your projects/jobs, it automatically stopped the timer if you are away from the computer, you can produce invoices with it and keep track of who owes you $, you can add expenses, etc... And there's a little app for your iphone so you can still track time if you are away from the computer. Like you could have a meeting and you could turn on the timer on your iphone and stop it once you are done with your meeting.
I've also used Freshbooks for someone else, but it just blows.

i have to use harvest at work, but i find a pen & paper more useful because adding 4 minutes here, 3 there, doesn't really make sense. you should have minimum sets of time blocked out and round up the next hour.

I use a free app now—timeEdition—because I couldn't always rely on the pen & paper method, and if I put it off for more than a couple of days I'd start to forget. Just pick the client/project and click the play/pause button.

I work for a company with 25 people and Harvest is the best time tracker I've used but I don't have to pay for it so I have no idea what its costs.

I used to diligently track my hours (agency & freelance), but I slowly found out that it simply doesn't matter that much.

I use http://crisply.com/ with Google Apps. It automatically predicts your timesheet after you train it to identify patterns in your email. I'm on a free version (5 or less users) so I don't know if it's cost effective for larger teams.
