I got a birthmark on my left calf, and I want to cover it up with a tattoo. I'm a tattoo virgin but I checked out a few shops for estimates. They said I needed to come up with the artwork so looking for some inspiration. I like the look of the yakuza tattoos.

Themes I like are strength, water, scorpios and resilience.

If the goal is a coverup then avoid themes that are super particular or specific. Opt for a decent design that fits the cover instead. I went over an older self done one on my calf with a basic americana rose. Works lloke a charm

Why cover up a birthmark? This is what makes you unique. I would get a tattoo of your birthmark on your other calf...
Just make sure you trust the artist 100%, I was lazy, and now I want to do some cover up work as well....
- Coving up something 100% unique with something someone else could have to fit in. Don't do it!sem

seen some good demon mask tattoos on calfs, they stand out from far, theres a lot of different kind of demons kinds/meanings, like tengu etc
check em out, there's a lot to Japanese tattoos/techniques, if you can, get a Japanese dude to do a Japanese tattoo.
They use different inks (flatter colors), special techniques in their line separations, things like that, they do look flatter (compared to normal techniques) in the beginning, but end up looking better and lasting longer in the end because of the japanese techniques.my dude from 12-ish years ago 'Horiryu' from Tokyo is based in LA now, but he does tour the US and EU a lot, hes hard to book now but if you can do it, do. according to his apprentices, back in Tokyo he tattooed a lot of the Yakuza bosses personally. hes extremely heavy handed though, dude was carving in my bones.
another guy that did part of one of my sleeves is Horishow out of Fukuoka-shi, Fukuoka, Japan, he has a way softer touch, he doesn't get out of JP much but he does tour on occasion.

Is the area behind the knee more painful?

^
i havent done that, but from my experience, any area that isnt exposed to much, like around the arm pit are tender as fuck, or areas that have a lot of tendons like the elbow swell up big time.

Behind the knee will be a bitch with the healing process though because you'll be bending the area lots so will most likely need to go back for touchups also.

Spend some time poking around http://www.yellowblaze.net/.
A lot of it is traditional, lovely and quite inspiring; also the link above (sets).

If you're not in the Yakuza, don't get a Yakuza tattoo.
If you are a creative person, make your own design. Or put some album artwork / reference something you really like.
Don't have some tattoo artist draw something for you in their style.
