Also, add _README files to toplevel folders explaining file organization and naming.

My own personal folder structure is usually something like:
Client Name > Project Name,
... in which you will find:
00 Assets > 00 From Client, 01 Gathered
01 Planning (For pitches, cost estimates, research, etc)
02 Concept > Concept 1, Concept 2, Concept 3, etc
03 Production > css, images, scripts, etcI've used this structure anywhere I've worked, and my colleagues have always appreciated it.

This is the method I prefer...
Top-level Folders:
3-letter client name
All four digits of the yearProject Sub-folders:
- Internal project number
- dash (don't use underscores, use dashes with spaces around them so you can jump between words if needed for renaming, etc.)
- Short name (New Corporate Pocket Folder)Assets within project folders:
- _assets (logo files, source materials from client like head shots, etc.)
- _docs (any docs you need to have locally; e.g., copy for flowing)
- project-type folders (www, psd, staging, etc.)So, for example:
- dbloc0
everything should be named untitled.
start with untitled1 and just increment up as you go.

Why don't people just sit down at their desks and think a problem through? Nowadays they post on a forum and then people send links in to articles where know-it-alls have got on their high horses and written up blog articles on their methods - which aren't going to work universally as they are too tailored to a specific circumstance
The internet is getting full up with blogs full of know it alls talking about best practice. I do wish they would shut up.
