Version s/be padded with zeros for sorting but date instead than version might be more informative, date format should be YYMMDD, '-' is easier to type than '_' so project-worker-date e.g. proj-abc-100513

I tend to call it "file-name.aspx" according to what the file does and then let svn take care of client, projects, versioning etc.

from my experience, putting dates in files, ALWAYS goes wrong .. other people work on it .. don't resave .. date doesn't match anymore .. versioning problems etc..

yeah, when looking for a file what do you start searching for ?

we ned the TV spot of XXX that we did last year , sometimes we do not remember exact dates..maybe we start searching by client, proyect...then we need to find that final version

If I make more than one revision on any day, then I'll use v1 etc after the date.

This is typically how i set up a file structure. I've found it works quite well on an individual level and moreso for me personally. However, I will warn, that as soon as other designers get added to the mix it can get to be very confusing / disjointed.
001 Assets - Pretty self explanatory
002 Concepts - This is where conceptual/WIP documents are store/created. I typically label them sequentially starting at 000 and will created as many 'versions' up until what is pretty much finalized artwork.
003 Rounds - These are simply for presentation purposes. I will store the WIP file from concepts as well as any client-facing file (PDF's, jpegs, etc) in a sequentially labeled folder. I add what the round was for and also stamp it with the date.
004 Working Files - These are production ready files for print or web.
005 Management - Will contain anything related to invoicing/rfp/rfq/creative briefs
006 Specifications - If the project has a particular set of parameters or specs to work with any docs that outline this are stored here.
007 Recd from client - Typically client communications and/or feedback.

I knew a guy with a really loose system, most of the time just named files based on the first thing that popped into mind when saving.
They lost a client when they sent them something like 001_Client_Round2_Buttsecks.pdf
- guy who works here did that not too long ago. Something like "001_not_feeling_it" and sent it off. Client was like "WTF"d_rek
