  My own personal folder structure is usually something like:

  Client Name > Project Name,

  ... in which you will find:

  00 Assets > 00 From Client, 01 Gathered
  01 Planning (For pitches, cost estimates, research, etc)
  02 Concept > Concept 1, Concept 2, Concept 3, etc
  03 Production > css, images, scripts, etc

  I've used this structure anywhere I've worked, and my colleagues have always appreciated it.
