forget the fine hair, it's not worth it. make a rough selection and work the edges with the heal brush

Use the pen tool, don't bother with the fuzzy bits.

all those tools won't give you the perfect result, you will still get bad edges on fine hair and you will still have to tweak them manually -> let it go

i remember going over a few tutorials for doing this over at lynda.com - i don't have time now, but can look it up later - you can get a 30 day free trial on that right? might be worth checking out, i love that site.
