It's a traditional part of cafe / pub culture and European non-smokers seem less zealously preachy. Also, some parts of Europe are so horrible you might as well smoke yourself to death.

I think it's probably not that they smoke more in Europe, it's that travellers smoke more.
Most of the Americans I know living in the UK smoke, where most of my UK friends don't. I've had this discussion with US friends and they've found that foreign friends living in their country seem to smoke more than the locals.
Another traveller trend I've noticed is homosexuality. All of my gay friends are foreign but I don't think I actually have any close English gay friends.
Conclusion: Gay people and smokers travel more.

seems to me smoking is a _human_ thing and that only recently has it fallen out of fashion in the US... judging from media my guess is that early/mid last century for instance most men smoked. look at mad men for instance (not a source of historical fact of course but it represents an accepted cultural memory). those fucks smoked non-stop. i'm guessing the whole politically-correct movement in the 80s/90s here influenced the whole "omg i'm so offended by your smoking" thing which brought about new laws 'n' shit
