Does anyone bother applying the PNG fix for older browsers anymore...or do you just assume that PNGs will work?

Don't bother. It's only IE6 that has issues with it and unless there's a specific need to cater for that wretched browser, then it's safe to ignore it.

if your client is paying extra for it then just serve them gifs and dithered to fuck.
If they're stupid enough to still be using IE then their computer will barely be powerful enough to run most modern JQuery drenched websites anyway so they'll be used to things looking broken, slow and shitty.

i created and use a boilerplate and have the pngfix incorporated into it. can be easily removed if i feel the website doesn't need it. no harm done.

yes, unfortunately I still occasionally need to support ie 6 & 7 (background transparency for 7) but use Jquery PNG fix:
