what size document should I create when starting to design a logo. The primary use would be for web but it will also be for print.

if you start in vector format, size isn't really relevant.

8.5 x 11 piece of paper and a pencil are the best document set up to start a logo.

Odd request, but can't go wrong with a 4" square.

as F_B says, size isn't relevant if you make it as a vector, which you should try to. What you do need to pay attention to, if it's primarily for web, is how it looks at webscale resolutions and sizes.
Easy enough to do —just play with your zoom level in Illustrator or whatever program you're working on, and imagine it set at a ‘worst case’ scenario, tiny and greyscale, lost in a footer somewhere, taking up a mere 100 pixels or so width on-screen.

1mm x 1mm is best, so when clients ask if its scalable, you can say FUCK YEAH!!!

something that looks balanced on a standard Letter/A4 works for me... actual dimensions is somewhat relevant to shape of logo.

... and make to sure to shutdown your computer before you turn it off.

scale it to whatever you're presenting on.

Create a standardized template and work off that... mine is an 11x17 with my logo and various other shite on it... with a large open window to work within.
