Anyone have any real suggestions? What worked for you?

i've always threaten to leave, that has worked so far...

being an asset to your company usually works then something along the liones of; " over the past xxx months i've been more than succesful on numerous projects, so-and-so client was especially good for this, this and this reason... The problem is to do the level of work on a month to month basis takes an incredible amount of time and effort on my part and something I feel my current salary level doesn't match.. blah, blah"
OR
" SHOW ME THE MONEY!"

blackmail usually gets you what you want.

Bit of what _me_ said and the first post too.
'I've done this, improved in this, helped win this, and I've been here for x. I'd like more money please.' If no then hand in your notice, that usually calls peoples bluff. It's worked for me twice.
At the end of the day if you feel you should be paid more and they won't pay you more, it's a pretty unworkable position. As often happens people outgrow a company and move on. I've always used the lack of rewarding my performance (if it's been good) as an indicator that I might be ready to move up the ladder. i.e. middle designer to senior designer and so on.

if your company has regular preformace evaluations just bring it up during your evaluation, espicially if it is going well.
if not just go balls out and ask for the raise, you might get a negitive reaction at first but it you are really worth what your asking once your boss thinks about it they will realize you are worth it, and in the end they will be be happy that they did the right thing and you will be happy you got that ca$h

i got 20% but I had a list of improvements that i've made since i started. reducing waste, improved client relations, made this more efficient, stopped wanking off in the bathroom....u know.
good luck!

Interjecting Beastie Boys lyrics will do wonders for you.
Especially if they turn you down, you stand up and stare them down and say "No time like the present to work sh*t out!"

truthfully, ive gotten good raises because :
A. performace reviews are always outstanding
B. i try to create work that is an asset for the company, even on big team projects. i try to put my own spin on our work, so when the raise comes up, i can take more personal credit than other staff.

remember that a raise is the *easist* money you ever make in your career : if you were to take a year's (or however long) worth of the raise money and stuff it into the hour you spend with your manager, thats the most cash you'll ever make for an hour of effort!
anyway thats how i screw my courage up before i walk in..

I got it!... a 22% raise in pay! Made a list of clients and what I've done for the agency. He didn't even haggle. It's gonna be a good year.
