this has bad idea written all over it. I don't think I've ever clicked on a banner (at least not on purpose), and sure as fuck wouldn't fill out a form in one.
But depending on the type of ad you buy, you could have the banner expand on mouse over - Did that a few months ago for a campaign.- < +1********
