    Please accept this...

    Dear XXX is only suitable if you know a name, and it has to be the RIGHT NAME. Sir/Madame sounds hackish, you might as well say 'dear whoever happens to read this letter'.
    By stating Attention XXX department or manager will at least get your email/letter routed to the person making the decisions, and it doesn't leave much room for an insult.
