I scanned a picture of an illustration for a post card when I worked for a printer. As a joke, I put "Tippy in Concert" in graffitti font on a wall in the image. Tippy was the guy who cut stuff in the back. He looked like Ron Jeremy and eventually died of heroin. It was still on a separate layer when I gave it to the pre press guy. He flattened it without looking and it went to press and out the door without anyone knowing until the client called the owner to complain. The owner blew a shit fit but realized how it happened.
