I used NO Explode for a while... one of my favs... but I had the same concerns. I didn't have any health issues... although when I drank a bottle of Redline I thought I was going to explode. That shit was terrible. Heart racing, sweating... terrible.
I take Animal Pack Rage (literally just started), and really like it. Doesn't give me the "caffeine" style crazy rush, just more focus and a bump on pushing myself. I take about 40% of the normal dosage and only on weights days.
I do have similar health concerns and I dropped pre-workout drinks for a while. I found when I was super strict on my diet I had great energy, but that meant no drinking, no refined sugars, clean carbs... just really tough for me since I'm not a body builder.
In the end you're asking a design forum, so I doubt you'll get official medical advice, but a lot of people like me and other above, have taken it in the past. So good stories and ideas :)

My wife's trainer had to go to get a liver biopsy because she was taking too many supplements. I guess its all about moderation.
I drink is protein shake after workout. Not the healthiest guy around (still overweight), but it helps curve my appetite in the mornings.
