...when a client doesn't like an artwork and wants you to create a new one from scratch - then rejects that one and asks for a fucking different concept?
It's never happened to me before so I don't quite know how to react.
I don't have a contract as it's going through an agency.

Unless you need the money for rent, walk out. Be really respectful to your agency contact, but just walk out. There's no contract. In the time this ass is rejecting your concepts you could be working with a client that likes your work and pays on time.

don't waste your time,
your client is just trying to see if he can make you work for free,
test him by saying, "every new artwork I do from now on will be billed as new projects"
and watch him run

Thanks guys. That confirms what I was already thinking.
I'm actually going to do it as the payment is not bad - but am being very clear that this is the last one that I'm doing.

It depends. If I was being paid freelance rates by the hour, I'd rejoice (whilst being a little frustrated, naturally). If it was a one-off payment, I'd tell the client they need to pay more, as they had clearly approved that concept (right?).

I would say, you happily bill your client for a new concept, even more, I suggest more than one to make sure he finds something he likes.
Of course you are meant to be PAID for all that, aye?

Always a good idea to get the client to sign-off and pay in stages.
