Can not turn brain off.
How can I focus and achieve bigger goals? My thoughts paralyze me.
I don't even know why I'm posting on here, I'm almost desperate for a way to slow my mind and limit the scope of my concern to that which suits my values and goals. In this state, I envy the obsessive.

write a list. Follow it.

write all your thoughts onto post it notes. post them to a wall in your office. organize them outside of your brain. focus on important ones.

Lists are good and don't be afraid to add to the list just get everything from your mind onto the list so you can give your mind some breathing space.
Next, (actually its something I learned from someone on QBN I don't remember who but it was in a thread of a similar topic), is to separate what your top priorities are from what is important or interesting but not a top priority. Separate stuff that you want to do but should just sit on for a bit. Its not as easy as it sounds but it'll reduce the real list of things to do.
I have a large notepad document that just keeps growing but its separated into at least 3 sections of priorities. Sometimes I think I'll never get to all these things but I know as long as I keep at the stuff at the top the other stuff will reach the top or simply fade away into the stupid idea I wish I never had pile.

^another good thing about separating the list - do some of the non-important/easy stuff first. it helps morale when you see stuff getting checked off, and puts you in the right mode to start tackling some of the bigger stuff.

^Thats a good point, I do that a lot as well. I'll just get sick of doing one thing and will find myself one day feeling completely uninspired and can't even move, just staring at the screen, but then I realize there's this small but fun and easy project I thought of a few months ago sitting in the list and I immediately get the juice to start it because it fresh. Once I get it out my system the other more important stuff inevitably becomes more interesting to work on.

Exercise is great for focus. Concentrate purely on your form and work hard enough to get a good sweat going. Have a quick cool shower, and then you'll feel like you can achieve so much more.
Fitter people achieve a lot more than couch potatoes.
