it might help to post what the original cd art looks like. i'm gathering that from the render, those splotches of grey are supposed to be solid white?
start by putting your refraction down to 1. plastic jewel cases don't refract light in the same way a glass vase does.
do you have a global ambient occlusion setting on or global illumination?

Definitely, let's see that artwork alone as a 2D bitmap.
What exactly are you referring to as "patchy". The gray/white shifts to the left of the hair artwork?
Shadow noise can be reduced/smoothed by increasing the "Maximum Samples". In GI, you can increase the quality under Irradiance Cache by increasing the Record Density from "medium" to "Delaunay High".

I'm curious too what that poly looks like that the image is mapped to. If it is not perfectly meshed, it'll cause odd poly refraction.

Man, the more I look at it, it looks like 2 materials are overlapping in the same space. You using plane selections and assigning multiple materials to one polygon shape? (The sleeve poly)
