Here's a simple formula...
* Estimate the # of hours it will take you for design + revisions.
* Charge that # times your hourly rate
* Write a contract that spells out design options, including revisions
* Have client sign itDone.

I'm with sublocked.... I remember someone telling me take the hours you *think you'll spend, then multiply by 2.. but depends how well you know yourself and your client.
Other than that, you could have a set price per icon with a set number of revisions, then a price per revision there-after
