yeaaa that's my usual routine and I guess I'm not expecting anything amazing, but it does seem like it must have been a necessary thing in the past so I was hoping someone had a secret stash of amazing forms or something.

I've done plenty of forms for Accountancy, Financial Planning and Banking Forms.
There are two users to think about when designing – The person filling it out, and the person keying that data into the computer.
The best trick is to avoid 'hanging chads' :)
