registration and/or payment forms

Has anyone had to 'design' registration/payment forms in print? I'm with an organization that still does things by post because not everyone has regular internet access. We basically have to design a new one per event, which is about four or five a year. I just don't feel like I'm making any improvements and I'd like to see if anyone knows any good examples to take cues from.

have a cup of expresso, sketch the form on a napkin. make the thing in illustrator or indesign. done....volia!

yeaaa that's my usual routine and I guess I'm not expecting anything amazing, but it does seem like it must have been a necessary thing in the past so I was hoping someone had a secret stash of amazing forms or something.

I've done plenty of forms for Accountancy, Financial Planning and Banking Forms.
There are two users to think about when designing – The person filling it out, and the person keying that data into the computer.
The best trick is to avoid 'hanging chads' :)
