Web feedback form

  Ok, this is a little bit vague as I don't know what exactly kind of form this is, but I'm looking to create one of those forms that sits on the edge of your browser window. Anyone have experience with creating these? cheers.

  (I've googled the shit out of this but cant seem to find any info on this.)
