placeholders?

When designing/building a site, what do you use for image/video/media place holders and/or for advertising place holders.
This is really for visual research.
I commonly use the old square box with the crossed lines through it.I want to see examples like this :

I pretty much just use the box with text description in it ala #2 above. Sometimes if I'm feeling adventurous I'll put dimensions in the box.

- grey box
- sometimes grey box with line through it
- box with faded copany logo in it (can also be used for "no image" on live site)

an evocative image of what it might be (usually scraped from google) with the words FPO (for placement only) screened on top of it. Helps clients imagine...
- i have found that this is always bad because clients get hung up on what the image is.... even thought it is just a place holder********

make a player dude- take a screenshot, make your own buttons, put in FPO shit, fake ads &c
