Two different paths to campaign.
1)
Brand picks the photographer based on who they want the brand to align
- hires agency to determine the over all creative direction
- photographer works with agency to define the style and photographer pics models
- photo shoot (with agency to determine what types of shots are needed, lightbox, spread, lookbook)
- agency takes the retouched photos and adds typographical treatment
- photographer has final cut2)
- Brand asks for new campaign
- agency researches mood / look / models / photographer
- agency determines all creative direction but photographer maintains his/her signature look as thats why they were picked for
- agency is god (tells brand what to do) a la Baron & Baron

BUT having said that, there are photographers who picks treatment, typography etc if the agency is not well known and there is a completely new direction in brand. And photographers being well known.
