Quote Form not getting enough clicks...

Hey guys,
I was wondering if you could help me out. I am working on a website for a promotional company selling pens, tote bags, t-shirt printing, etc.... I have this quote form that I generated through wufoo. I have embedded javascript into my own page so that way the user isn't taken off the site.
According to google analytics, 900 people accessed the form this month, but only 277 people completed the form. Do you guys have any idea why there is such a large gap of people leaving the form?
Any feedback is appreciated. As a side not I left out the company logo which is at the top of the form, and there is a border at the edge of the form which corresponds to the companies colours. I left that out from that picture because I wanted to focus on the form itself.
The only thing I could maybe see is that there is a java script issues?

sounds like a pretty standard bounce rate for any page. everyone clicking on the link or reaching the page might not be there by intent of filling out the form, finishing it or even look at it.

^ agree, i don't think the form is failing per say but that # sounds average

30.8% completion isn't too bad.
Remove every input that isn't absolutely necessary.

isnt average bounce something like 85%

Yeah, I mean that does make sense. Does the fact that it calls up javascript to load form cause any issues anymore? I would assume all computers would be able to open it.

I would understand the bounce rate being that high if it would a page you come across in google, but being that it is a call to action button internally on the site, I would assume it would be a much better rate. Maybe I am wrong.

Is there too many required fields?
Right now there is:
Name
City
Email
Postal Code
Product Name
Product Quantity
Description of work to be doneThat was brought up by my boss, I know its information we need, but do you think its too many required field. Would that turn you off?

I noticed on the call to action button one of the options listed was to "place an order" seeing a form immediately made me feel like the buying process was going to be drawn out. That may account for a few.

Actually I thought they were combined, but they are separated in analytics.Request a quote was 70% abandonment rate
Place an order was 96% abandonment rateTotally shows what you mentioned about it could make the user feel like the process is drawn out.
