yup, web designs look so generic these days. chances are you can find a doppelgänger of any site. make the copy site but charge the same amount as you did the first time, else client A is paying for client B. and spend the extra time and effort servicing the client. help them with their SEO, marketing, offer them content advise. And as Amicus says, minor changes will invariably make thing look quite different.

In my experience you do it exactly as they first ask, it won't look as good cause the content will be different and then they'll make enough changes so it looks nothing like the original.
