My company has built up quite an industry niche over the past 5 or 6 years. Our portfolio is pretty decent now with close to 100 sites on our books.

Over the past year there's been a bit of a trend with new clients approaching us and saying

Client: "give me one of those".
Me: "You mean something with similar structure or colouring?"
    Client: "No...something exactly like that"

As much as it makes brilliant business for us, there's obviously a bunch of issues that need to be dealt with.

My business side says "just take the job but don't include it in the portfolio". My other side says "tell them to bugger off".

In this environment, it's difficult to turn away projects.

In my experience you do it exactly as they first ask, it won't look as good cause the content will be different and then they'll make enough changes so it looks nothing like the original.

yup, web designs look so generic these days. chances are you can find a doppelgänger of any site. make the copy site but charge the same amount as you did the first time, else client A is paying for client B. and spend the extra time and effort servicing the client. help them with their SEO, marketing, offer them content advise. And as Amicus says, minor changes will invariably make thing look quite different.
