yeah, i draw it on paper. then i sketch it out more precisely. then i trace the sketches on tracing paper. which i scan and adjust the contrast. then i send it to a specialist to draw out the type more precise by hand or machine. that i will scan again, enlarge, put on the wall, look at it. evaluate it on many levels, intellectual and emotional. then i will have several meetings with art directors and creative directors. when they approve the scans will be opened in illustrator, there it will be traced to vectors, then anchorpoints and paths will be optimized until the type looks perfect, then a screenshot will be made (it is for screen use only, so that will be good) which will be opened in photoshop and optimized for web. it will be sliced and then by a front end designer converted to divs..
anyway, if all gets rejected it's back to the drawing board again.well, some websites have photos. that's a different story. it requires weeks of research and fierce analysis and a global round of portfolio browsing and finding the right photographer, sketching ideas, create storyboards/moodboards, do location scouting etc. etc. until you and the photographer get that perfect shot for the header of the website: a beautiful representative lady in a clean offfice environment talking over a headset with a smile.
