em, but why not use them where they are required rather than based on personal preference?

I like both space en space, or space em. use whichever /looks/ right in a specific typeface or point size. Sans faces tend to get the en dash treatment.

Either, but pick one and stick with it. I prefer the em dash—no space for parenthetical statements. I use en dashes with no space for dates, e.g. 1998–2003.

I often interchange question marks with exclamation points, or vice versa. It's a style thing. Forget proper grammar, I go with what looks better.
