PDF Form, how too?

Can somebody advise me on how I can do these, I have a client who wants to fill the PDF form I have designed for him using boxes created on the address, telephone, email details then email it to a client for printing out and signing.
Something very similar to this one:
www.cml.org.uk/cml/filegrab/Disc…...What programme can I create these in, all I want to do is create the boxes so he can fill them in using acrobat reader.

you can do it with any doc and acrobat professional. the client will need acrobat professional as well if they wish to fill out the forms.

Johndiggity is right. Just show the 'forms' and 'advanced editing' toolbars and you shouldn't have a problem figuring it out from there. If you need a hand, send me the form and I can do it for you, or at least get it started.

You dont need Acrobat Pro to fill out the form. That would defeat the purpose of creating a form where anyone can fill out.

Their used to be an issue with the software only being available for PC. Did that change..?

it used to be that you could fill it out and print it, but not save it locally.

I'm filling in a contest entry form where they require a signature but insist it must be return in same pdf document - if the original pdf is altered the form is void.
Fucking thing seems like a scam...
