Silhouette was the old plugin I was trying to remember with the centerline option. If you're still running CS3 it looks like the demo is compatible.
Rasterize your shapes and retrace with centerline option. Won't be perfect but I think the best automated result your going to find.

oh wow i just tried the same with Live Trace.
if you crank up the stroke weight option it actually does recognize a centerline.
decent results too.

actually LiveTrace results are kinda crappy on shapes other than basic boxes... but still, it did do what you requested:
