Has anyone ever had a price quote request for a "very simple website", then after sending them the quote and getting the project started, they add extras.
I had a request for a "simple, sleek website". Sent them a quote, started the project and now they need custom illustrations, wild Flash animations, etc... None of this stuff was indicated in their original plans for the site, before they asked me to price the project.
They're expecting me to add all of this extra work without adding to the price.

Its called feature creep. You need to let your client know its out of scope in relation to the original price. Let them also know that you have no problem doing it as long as they uderstand it needs to go through a change order process and a requote.

Of COURSE they're going to take advantage of your fixed rate if you let them.

I do have a contract.
Their requests are so far out of scope that I'm not going to deal with them. I'm refunding their deposit and hopefully they'll be honest with the next person they hire.

If you've started working, that's partially yours.
And don't just dump them, bad move. See if they are willing to pay first. If not, then dump them.

Pay attention before you do something drastic. No need to cancel it because they want more work. Tell them you have a contract and negotiate the rest with them or tell them to wait until a later phase.
