No dilemma that I see, although I see how one might worry.
Someone needs to share this guys load while he's rehabing. He should understand that needs help.
I'm sure the guy would appreciate a call or email at least so not seem as you are going over his head or trying to cut him out.

Update,
The print rep I mentioned brought the designer by our office to meet us just now. He said he's about 90% and was doing well. He looked and seemed good. He thanked us for the offer and is on press all day with a big job. It was good to meet the guy. I'm glad things are coming full circle for the fella.
