rope swing on a camping trip...
I had slack in the rope and didn't realize it. I jumped off the rock into mid air. When rope caught tention, it snapped me off dropping me 5 feet onto some large rocks, landing on my side and back.
I slipped off the rocks and into the water, where I was supposed to land originally. A 2 hour hike out of the woods to get 7 stitches in my hand was the result. And yes, there were a lot of witnesses.
It sucked, but could have been a lot worse.
