have u tried to break up in Post-It? (reminds me of SITC).
It will be hard, there's no way of facing good breakups.. who knows she might do things, or try to see her mood first before you do.
Don't ever breakup with someone when that person is having stress and pressure. It will lead to 'weird' doings... as such you guys know...
