Your list didnt include anything about creative skills (or being a design graduate)... I think too many people generalize in this field, when they should be defining designer/developer/programmer/te... roles more precissely (even if these do overlap a lot).
We're a client (with a very small budget), and we are looking for designers who understand web standards, usability, CSS stuff etc, and we're looking for programmers who can also format a page nicely without everything having to be defined for them... But we dont expect to find the full range of 'look and feel' creative design through to PHP/MySQL programming and server admin in one person. We can't afford such amazing talented people, if they do exist, and believe we are better to break these areas down and employ a few people part time instead (it's also safer in terms of not commiting ourselves to a single, untried, employee).
