The one that's spreading right now is adware based, but there has been a worm that spreads via email first then attach itself to email contact list as well as IM buddy list---and they are actual virus. Most of them so far have been pretty low-risk, simple nuisance of showing some porn or popups or stealing your IM profile info (which is available mostly on IM directories anyway), but eventually someone will figure out very pervasive worm that will spread via IM.
Simple precaution and constant updating of your anti-virus definition would prevent most of such attacks.
