A: Don't work hourly. Charge by project rate. Your efficiency should benefit you.
B: Include all of the stuff that's not actual creative work in your estimate/proposal. This includes an estimated cost for meetings, phonecalls, invoicing, coming up with an estimate -- anything that requires your time and is related to the project is time you should be billing for. This is what a project manager / ae/ finance person has to do at a company -- and they don't work for free, neither should you.
