Based on a leaked script, the last episode shows that this was in fact only in Hurley's mind after being in a fried chicken induced coma.
He awakens, disoriented and hungry. He sneaks out of the hospital to find something to eat. As he stumbles about the city streets a kindly strangers asks if he needs helps and if knows where his is going. Hurley stares into the camera and says "I'm Lost".... BOOM. END.
