Surely if you're writing a letter, then it's a formal communication by default. Forget any email- or other 'modern'- style salutations as you'll sound like a dick, like some of these ^^^ : )
To whom it may concern is formal, but if you can get more specific with a job title can't you?
Personally I'd go Dear or Attention [title] and keep it gender independent.
