Contact form worthwhile?

People need a reason to give you details.. I don't want to be contacted by companies unless there's a good reason... I reckon it's the same for most people.
* If you have a contact form attached to each out-of-stock product in an ecommerce site ...I might fill this is in so I'm emailed when my product in stock.
* If you have a mailing list which offers regular discount coupon codes for the site, and I'm interested in the products ... I'll sign up using the mailing list contact form.
* If I want to contact a company, I like to email - because then I get a record of the conversation with all my other emails.
If you're going to offer a contact form -> I'd suggest you ask for the bare minimum of details required.. or at least make the extra fields optional. Just put yourself in your site visitor's position and you'll okay.
