When I was there about 9 years ago, we traveled around quite a bit, but stayed in San Jose, Tortuguero, somewhere near Arenal, and a couple other places I can't remember the names of. Traveled mostly by boat, which was really cool, except most of the rivers were totally out of control due to some wicked monsoons. Thought I was going to die more than once. It was excellent. :)
Was a bit freaky when we got stopped at by border patrol in Nicaragua while on the Rio San Juan (i think?). The guards wanted a us to take a picture of them with all the women in our group. Who were we to tell a bunch of guys with big guns they couldn't have a picture? :)
Tortuguero was fantastic. Got to see giant sea turtles laying eggs on the beach and did some wicked hiking through the rain forests.
I highly recommend doing some sort of canal tour if you go there (in Tortuguero, that is)
