Ok, you guys are the f'n best. Thanks a ton for the discussion, too, because i've asked so many people and keep getting different responses. Here's what i have:

    1. DIRECCIÓN DE DOMICILIO (SI ES DIFERENTE)
(because I have a DOMICILIO field above this one)

    2. TIEMPO COMO PROPIETARIO
(This means "time of ownership" instead of "duration...". Is that okay, translation-wise?)

    3. RETENCIÓN DIARIA SOLICITADA (PORCENTAJE DE RECIBOS DE TARJETA DE CRÉDITO)
(someone else had it as RECIBO, without the "S", but i assume theirs was singular and it should be plural.)

Length of ownership should mean, "How long have you owned this business?" Is everything correct above?

Thanks again for all your help, guys. You're the greatest.

    That all sounds about right.

But regarding the 1st one....I don't get that. If you are using the word "DOMICILIO", you don't need to use "DIRECCION DE DOMICILIO" below it. that'd be like asking on one field for a phone # and on a second field repeat the question but using instead of the word phone, "that small box with tiny buttons ranging from 0 to 9 used to communicate with other people not present.

Anyway, if you are asking for a HOME ADDRESS (DOMICILIO), you shouldn't be using DIRECCION DE DOMICILIO, coz it's the exact same thing. Perhaps you are refering to a MAILING ADDRESS. If you are asking for a MAILING ADDRESS, this should say something like DIRECCION POSTAL.
