Witt's right. Every situation is different. If we really need to be aasking all these questions then you're not ready for this relationship again (and anything else until you figure some more things out, imo).
Also, I'm with para about how I've never seen broken-up couples come back together successfully. I've seen many people attempt it, but they always break up like 10 times and get back together over and over again until they realize how unhealthy their relationship is, or just grow up.
I don't know everything about you or your gf though, so I can't really say what you should do. Basically, think about it for some time and let things clear up a bit before diving back in, should you decide to.
