If you're not VAT registered, your invoice is essentially just a quantified list of services and relevant charges. You should include invoice date, a unique reference (or job number + date etc) and provide a totals column.
Looking at what I've just written, I'm sure I'm telling you nothing you don't already know, so here's a link to an old one (for a VAT reg'd company, so just ignore the VAT total section).
