it depends on the fabrication. you can do your own development and have it done by a factory, but you better have big orders to cut. if you are doing it domestically you can do smaller quantities. you can also sew by yourself if you want/have the equipment(industrial sewing machines if you want it done right).
i also forgot one alternative: acid sublimation printing, which can go all over if you get a good printer that knows what he/she is doing. you also can't do it on 100% cotton material, it has to be 50/50 or all synthetic.
apparel manufacturing is a tricky business and a long and windy road, not to mention extremely expensive. good luck!
