i do a fair bit of ui consulting, both from an interaction and visual design standpoint. i've been asked to join a few companies in permanent roles, but then you run into some of the problems you mentioned in your post. as a consultant, my role doesn't usually span more than a few months, at which point the internal team of a given company will take it from there. i provide a structure and visual style which they go on to implement in the actual product. the only real downside is the lack of control in the finished product, so more often than not, the designs are implemented in a not-so-optimal way.
