There is always a deeper level of thinking that goes on way before you act or react to seeing somone who's life is in danger. The reaction may seem "off-the-cuff", but it is ultimately a result of a well-formed sub-thought that aims to bring the best possible result to your own body or mind.
The reality of this thought and the response to that reality is summed up well in the second greatest commandment. "Love your neighbor as yourself"
