  I ALWAYS use an RGB TIFF, and I make the image typically an 8.5 x 11 bacground image in the header of the docuemtn, that way if they go to page 2, 3, 4, etc. it is there too!
  TIFF's print amazingly out of Word and the CMYK - RGB holds the best I've found. On screen, it'll look tinted, but that's because I place it in the header of the doc., it prints great.

  And yes, I always set Margins, Styles, Paragraph (leading), and drop in a sample letter with placement of blocks (To, From, Date, Body, Salutaion, etc.)
